Bookings can be made online under Health Assessments.

Appointments are generally 30-45 minutes.

Our practice nurse will initiate the process of the assessment.

  • Check ups are available for the ages

    • 1-4 weeks

    • 6-8 weeks

    • 4 months

    • 6, 12 and 18 months

    • Annually until 4 years old

  • Suitable for couples planning on starting a family.

    Please book a long appointment with doctor and nurse.

  • A comprehensive assessment of your health in the perimenopause or menopause.

    Includes , cardiovascular risk, bone health and mental health screen.

  • If you are over 30 without existing heart disease , you can have your risk of developing heart disease assessed annually.

  • If you have a risk factor for chronic disease and are 45-49 , book in for a full health assessment. A peri-menopause screen will be included for women in this age group.

    Anyone 40-44 is also eligible if at risk of Diabetes.

  • This is planning meeting intended to explore your goals with respect to having a chronic condition(s), and to co-ordinate care to align with your health goals.

30+ Annual Heart Health Check

Anyone over 30 years old without known heart disease could have a Heart Health Check.

A comprehensive health assessment is undertaken to determine your risk for cardiovascular disease.

You will spend at least 20-30 minutes having your cardiovascular risk categorized. This may include having a blood test and ECG.

Interventions such as lifestyle changes, or a referral to a cardiologist may be recommended.

45- 49 Health Assessment

A once only preventative health care consultation, provided to a patient 45-49 years old with at least one or more risk factors which can lead to a chronic disease.

A risk factor may include family history, high cholesterol or blood sugar result.

Our team will undertake a comprehensive information collection of your medical history, psychological and social function, undertake an assessment and then provide you with a written plan.

Consultation time: at least 30 mins required.

A health assessment to identify ways to improve your health and well being.

This thorough assessment is undertaken by the practice nurse and doctor, looking at your physical, psychological and social function.

It helps to assist with early care needs. You may be eligible for help within the home by applying to the government My Aged Care.

Chronic Disease Management

Already have a chronic disease?

A chronic disease is defined as long lasting medical condition with persistent effects.

Some well known examples are:

  • Hypertension (High Blood Pressure)

  • Diabetes (High Blood Glucose)

  • Liver Disease (Fatty Liver, Viral Hepatitis)

  • Kidney Disease

  • Asthma and Bronchiectasis

  • Mental Health Conditions (Depression and Anxiety)

  • and many more.

What is a Chronic Disease Management Plan?

Medicare provides Chronic Disease Management Plans (CDMP), also known as GP Management Plan.

The purpose of CDMP is to identify your chronic health problems, set health goals and then co-ordinate care.

CDMP provides you with ongoing care with a team of health professionals, including

  • 5 service visits per year from a Practice Nurse

If you are eligible for a Team Care Arrangement, you may claim with a Enhanced Primary Care (EPC) Referral

  • 5 Allied Health professionals’ referrals per patient per calendar year

  • EPC visits attract the Medicare rebate

Plans are reviewed by the GP every 6 months.

There are rules around eligibility for CDMP and TCA, so it is best to speak to your doctor.

A New Beginning …..

There is no better to time to check in on your health.

To provide the best outcome for your pregnancy, folic acid supplement is essential. Depending on your health, you may need to optimise your diet and lifestyle and other supplementation as well.

Preparing for your pregnancy includes preventative health care including blood tests, and cervical screening. It is recommended to have a health check at least 6 months early.

A screening test called Reproductive Carrier Screening (RCS) was introduced under Medicare to all prospective parents and pregnant women.
